FAQ – Efficient Implant Dentistry with the MERIDIAN® Implant System

Modern implantology should not become more complicated simply because more technologies are available. The goal is to make the right clinical decisions, reduce unnecessary treatment steps and create a workflow that respects both the clinician’s time and the patient’s biology.

The MERIDIAN® Implant System was developed around this principle. It combines a tissue-level implant concept, simplified prosthetic workflows and BIO-X bioactive glass-ceramic technology.

Below we answer the most important questions about immediate implantation, workflow efficiency, tissue-level implantology and BIO-X.





1. What is the MERIDIAN® Implant System?

The MERIDIAN® Implant System is a metal-free, tissue-level dental implant system developed by dr. Igor Dogocki, STEINSOHN Ceramics.


MERIDIAN® combines ceramic implant technology with a simplified surgical and prosthetic workflow. A central feature of the system is BIO-X, a bioactive glass-ceramic material designed to interact with the biological environment at the implant surface.

The system includes both one-piece and two-piece treatment concepts so clinicians can select the appropriate solution according to the individual clinical situation.



2. What makes MERIDIAN® different from conventional ceramic implants?

The most important difference is the material concept.

Many ceramic dental implants are manufactured from zirconia. MERIDIAN® implants are based on BIO-X bioactive glass-ceramic.

BIO-X is not zirconia.

The purpose of BIO-X is not simply to provide a metal-free implant material. Its bioactive glass-ceramic composition is designed to undergo surface reactions in contact with physiological fluids.

This gives MERIDIAN® a different biological material concept from conventional inert ceramic implant systems.



3. What does “bioactive” mean in relation to BIO-X?

Bioactivity describes the ability of a material to undergo controlled surface reactions when it comes into contact with the biological environment.

BIO-X bioactive glass-ceramic is designed to participate in ion-exchange processes at its surface. Under physiological conditions, these reactions can contribute to the development of a calcium- and phosphate-rich, hydroxyapatite-like surface layer.

Hydroxyapatite is closely related to the mineral phase of natural bone.

The concept behind BIO-X is therefore not simply passive biocompatibility, but an active interaction between the material surface and its biological environment.



4. Is BIO-X the same as zirconia?

No.

BIO-X and zirconia are different classes of ceramic materials.

Zirconia is widely used in ceramic implantology because of its mechanical properties and biocompatibility. BIO-X is a bioactive glass-ceramic derived from the BIOVERIT® material family and was developed around a different material philosophy.

The distinction is important because the biological behavior of an implant is influenced not only by whether the material is metal-free, but also by its chemistry, surface characteristics and interaction with physiological fluids.



5. Does bioactivity guarantee osseointegration?

No implant material can guarantee osseointegration.

Successful osseointegration depends on many factors, including patient selection, bone quality, surgical technique, implant stability, implant positioning, healing conditions, prosthetic loading and patient-related biological factors.

BIO-X provides a bioactive material concept intended to support favorable interaction at the implant surface, but it does not replace correct diagnosis, surgical planning or clinical expertise.



6. Is MERIDIAN® a bone-level or tissue-level implant system?

MERIDIAN® follows a tissue-level implant philosophy.

In a tissue-level concept, the critical prosthetic interface is positioned coronally to the crestal bone rather than directly at bone level.

The objective is to create a defined transmucosal transition and reduce unnecessary manipulation around the biologically sensitive crestal region.

For MERIDIAN®, tissue level is not simply a geometric feature. It is part of the overall biological treatment concept.



7. Why can a tissue-level design be beneficial?

A tissue-level design moves the implant-prosthetic interface away from the immediate crestal bone region.

This may help simplify the restorative workflow and reduce repeated manipulation close to the bone-soft tissue interface.

The concept is intended to create stable conditions for the peri-implant tissues while maintaining a clearly defined prosthetic platform.

The biological outcome still depends on correct implant positioning, tissue thickness, oral hygiene, prosthetic design and the individual clinical situation.



8. What is immediate implantation?

Immediate implantation means placing a dental implant directly after tooth extraction during the same clinical session.

Instead of extracting the tooth, waiting for healing and performing implant placement during a separate surgical procedure, the two treatment steps are combined when the clinical conditions allow it.

Immediate implantation can therefore create a more compact treatment pathway for selected patients.



9. What are the potential advantages of immediate implantation?

When properly indicated, immediate implantation may reduce the number of surgical interventions and shorten the overall treatment pathway.

For the patient, this may mean fewer surgical appointments.

For the clinician, it can create a more structured workflow by combining extraction and implant placement into one procedure.

However, immediate placement should never be chosen simply to save time. Appropriate anatomy, infection control, implant positioning and sufficient primary stability remain essential.



10. Is every extraction socket suitable for immediate implantation?

No.

Immediate implantation is a clinical option, not an automatic protocol.

The clinician must evaluate the individual extraction socket, available bone, soft-tissue conditions, infection status, anatomical limitations and the possibility of achieving adequate implant stability.

If these conditions are not favorable, an alternative or delayed treatment concept may be more appropriate.

Efficiency means choosing the correct treatment pathway, not forcing every patient into the shortest one.



11. Why can the MERIDIAN® Implant System be used in immediate implantation concepts?

Immediate implantation requires an implant system that allows the clinician to achieve controlled positioning and sufficient primary stability in the available bone.

The MERIDIAN® implant geometry and surgical protocol are designed to provide controlled bone engagement and allow the preparation protocol to be adapted to different bone qualities.

This can be particularly relevant in extraction sockets, where the implant often needs to obtain stability from available apical or surrounding bone rather than from the socket alone.

The final decision to perform immediate implantation must always be based on the individual clinical situation.



12. Is high insertion torque the main goal during immediate implantation?

No.

The goal is sufficient and controlled primary stability, not the highest possible torque.

Excessive compression of bone is not automatically beneficial.

The MERIDIAN® treatment philosophy therefore focuses on adapting the preparation of the implant site to bone quality and implant geometry rather than treating insertion torque as the only measure of successful placement.

Biology and mechanical stability have to be considered together.



13. How can MERIDIAN® simplify the surgical workflow?

MERIDIAN® was developed around a structured surgical concept rather than a large number of unrelated implant protocols.

The objective is to give the clinician a consistent treatment philosophy that can be adapted according to bone quality, implant dimensions and the clinical indication.

Reducing unnecessary variations in instruments, components and procedural steps can make the treatment pathway easier to understand and reproduce.

A simplified workflow does not mean less clinical control. It means removing complexity that does not add value.


14. How does MERIDIAN® simplify prosthetic treatment?

The MERIDIAN® prosthetic concept is designed to reduce unnecessary component complexity.

A universal abutment philosophy can reduce the number of components clinicians and laboratories need to manage. Selected ceramic abutments can also be shaped similarly to natural tooth preparations, allowing the prosthetic situation to be individualized.

The objective is to allow the clinician to focus on restoration rather than spending unnecessary time searching for different prosthetic components.



15. Does MERIDIAN® offer both one-piece and two-piece implants?

Yes.

The MERIDIAN® system includes one-piece and two-piece tissue-level implant concepts.

A one-piece implant can simplify the treatment pathway by eliminating a separate implant-abutment connection.

A two-piece implant provides additional restorative flexibility when the clinical or prosthetic situation requires it.

Both approaches follow the same basic MERIDIAN® philosophy: tissue-level positioning, reduced complexity and biologically oriented treatment planning.



16. Why combine tissue-level design with BIO-X bioactive glass-ceramic?

Because implant geometry and implant material should not be considered separately.

The tissue-level concept determines how the implant relates to the crestal bone and soft tissue.

BIO-X determines important characteristics of the material interface.

By combining a tissue-level design with a bioactive glass-ceramic material, MERIDIAN® aims to address both the macro-level treatment concept and the biological interaction occurring at the implant surface.

This is one of the central principles behind the MERIDIAN® Implant System.



17. What is the difference between biocompatibility and bioactivity?

Biocompatibility describes whether a material can function within a biological environment without producing an unacceptable biological response.

Bioactivity describes an additional material property: the ability to undergo specific reactions at the interface with biological tissues or physiological fluids.

BIO-X was developed around this second principle.

For MERIDIAN®, being metal-free is therefore only part of the story. The greater objective is to combine biocompatibility with a biologically active surface concept.



18. Can BIO-X form a hydroxyapatite-like surface?

BIO-X is designed so that its surface can undergo ion-exchange reactions when exposed to physiological conditions.

These reactions can result in the development of a calcium- and phosphate-rich surface with hydroxyapatite-like characteristics.

This surface behavior is one of the reasons bioactive glass-ceramics are fundamentally different from purely inert ceramic materials.

It is also an important part of the biological rationale behind the MERIDIAN® Implant System.



19. Why is implant material important in immediate implantation?

Immediate implantation places particular demands on the relationship between implant stability, bone biology and healing.

Mechanical stability is essential, but it is only one part of the process.

The implant material and surface form the interface at which biological healing takes place.

MERIDIAN® therefore combines a geometry designed for controlled primary stability with BIO-X bioactive glass-ceramic technology.

The goal is to consider mechanical and biological requirements as part of the same treatment concept.



20. Is MERIDIAN® designed only for immediate implantation?

No.

MERIDIAN® can be used within different implant treatment concepts according to indication and clinical conditions.

Immediate implantation is one important application because it demonstrates how surgical steps can potentially be consolidated.

However, the clinician should always select the timing of implant placement according to anatomy, bone quality, soft tissue, infection status and the overall treatment plan.

The system should adapt to the patient. The patient should not have to adapt to the system.



21. Does a simpler implant system mean fewer treatment options?

Not necessarily.

Simplification means reducing unnecessary complexity while preserving clinically relevant choices.

The objective of MERIDIAN® is not to remove clinical flexibility. It is to create a clear system in which implant dimensions, one-piece and two-piece concepts, surgical protocols and prosthetic components follow the same treatment philosophy.

A well-designed system should make the clinician’s decisions clearer, not create more decisions.



22. What does “efficient implantology” mean at MERIDIAN®?

Efficient implantology means performing every necessary step while eliminating unnecessary ones.

It means planning before surgery, selecting the appropriate implant and protocol, respecting the biology of bone and soft tissue, and using a restorative workflow that avoids avoidable complexity.

Efficiency is therefore not the same as speed.

A fast procedure can still be inefficient if it creates additional problems later.

The goal is a clear and controlled treatment pathway from planning to restoration.



23. Is MERIDIAN® simply an alternative to titanium implants?

We believe that description is too limited.

MERIDIAN® is metal-free, but the concept goes beyond replacing titanium with another material.

The combination of BIO-X bioactive glass-ceramic, tissue-level positioning, implant geometry and simplified prosthetics represents a different way of thinking about implant treatment.

The central question is not simply, “Which material can replace titanium?”

The more important question is, “How can material, implant design and biology work together?”


24. What is the main philosophy behind the MERIDIAN® Implant System?

The MERIDIAN® philosophy is to simplify implant treatment without simplifying biology.

The system is designed around three principles: respect the biological environment, reduce unnecessary clinical complexity and give the clinician a clear and reproducible treatment concept.
